If treatment services are obtained through an out-of-network provider, the policyholder must pay 25 percent.

With the Humana Basic 6350/National POS – OpenAccess policy requires policyholders to make the following copayments for diagnostic services:

  • $25 for PCP
  • $50 for retail clinic
  • $75 for urgent care
  • $75 for specialist

This plan provides great coverage for addiction treatment, with programs being covered at a rate of 80 percent as long as the deductible has been me. This applies when services are acquired through an in-network provider. If the policyholder goes through an out-of-network provider, he or she must pay 40 percent once the deductible has been paid.
