It takes care of 80 percent of your in-network inpatient treatment or 60 percent for comparable out-of-network care.
The plan also allows you to take 20 outpatient visits annually. After the deductible, it covers 80 percent in the network or 60 percent out of the network for the initial five visits. For the rest, it covers 50 percent with both in-network and out-of-network providers.
The Monogram: Total Plus Rx plan requires a $7,500 in-network individual deductible before covering the rest of your inpatient care. If you go outside the network, it covers 75 percent. For outpatient care, you pay the deductible and then the plan will pay for all of your costs for the initial five visits in the network or 75 percent outside the network. Others visits are covered at 50 percent for both network and non-network providers. You get the same number of days covered as you would with the above plan.
Another option is an Autograph plan. The Autograph: Share 80 Plus RX plan requires you an in-network deductible of $5,000 before it provides coverage. After that, the coverage is the same as the Portrait plan.
Finally, you have the option of the Autograph choices, which are the Total and Total Plus plans. These have different deductible options, and then they pay for all of your inpatient care with network providers or 70 percent with non-network providers. With outpatient visits, the plans pay for all of the care for the initial five visits in the network or 70 percent outside the network. The rest of the visits cover 50 percent for both types of providers. These plans cover the same amount of days as the other plans, and give you the option of a health savings account.
