All individual health insurance plans through your state marketplace give you coverage for substance abuse and addiction, including the ones offered by Humana. These plans also cover mental health and behavioral health concerns if you need the added treatment.
In Illinois, Humana offers different HMO and PPO choices. With the HMO plans, your insurance will only cover the providers in Humana’s network, which is a vast network of specialists and facilities. PPO plans cover those in addition to ones not in the network, but for a higher cost for non-network providers. There are numerous Humana plans available to you, with different levels designating how much of the care your plan will cover compared to how much you pay each month. The levels go from platinum, which costs more for more coverage, down to bronze, which helps you save money on premiums but covers less.

With this plan, you pay a 20 percent co-insurance both for entering an inpatient facility and for going to visits in an outpatient program. The PPO version of this offering also charges 20 percent in the network, and then charges a 40 percent co-insurance for out-of-network facilities and professionals.
Silver Level
For a silver level plan, consider the HMO choice of the Humana Silver 4600 plan. With this plan, you pay a 20 percent co-insurance for your inpatient or outpatient care after you pay the deductible. The PPO plan at the silver level has a different deductible. This Humana Silver 4250 plan requires a 20 percent co-insurance for in-network inpatient and outpatient facilities and a 40 percent co-insurance outside the network. There is also the Humana Silver 3650 PPO plan, which has no charge to you for your inpatient and outpatient treatment when you stay in the network. Go outside it, and you spend a 25 percent co-insurance.
Gold Level
A gold level HMO option is the Humana Gold 2500 plan. This plan comes with a 20 percent co-insurance when you sign into a rehab center or go for outpatient visits. The PPO choice on the gold level has the same deductible and in-patient cost as the HMO plan, and it charges you a 40 percent co-insurance if you decide to enter a treatment program that’s not covered by the network.
Platinum Level
The HMO platinum choice is the Humana Platinum 1000 plan. Like most of the other HMO choices, it has a co-insurance of 20 percent for both inpatient care and outpatient visits. The HMO plan is the only platinum offering from Humana in your state.
Catastrophic Coverage
You don’t have a charge for inpatient and outpatient services in the network when you use the Humana Basic 6600 plan. The PPO version of this plan provides the same coverage in the network, and also gives you the option to pay a 25 percent co-insurance for non-network providers.
