At the next level up, the Humana Silver 4600 PPO plan comes with a co-insurance cost of 20 percent for inpatient and outpatient care within the network or 40 percent outside of it.

The gold level offering is the Humana Gold 2500 PPO plan. In the network, your costs for inpatient and outpatient care would be a 20 percent co-insurance, while the cost to you goes up to a 40 percent co-insurance when you visit out-of-network providers. At the highest level offered, you can choose to sign up for the Humana Platinum 1000 PPO plan. This plan also has a 20 percent co-insurance for network inpatient and outpatient services and a 40 percent one for providers outside the network. With all of these plans, you might end up with a penalty for inpatient care unless you get it pre-authorized by the insurance carrier.

If you would prefer catastrophic coverage over one of the metallic level plans on the marketplace, choose the Humana Basic 6600 PPO plan. This plan comes with no charge for inpatient and outpatient services within the network and a 25 percent co-insurance for the same care outside the network. To keep from paying a penalty, you might need to get advanced approval for inpatient treatment.
