The Cigna Connect Flex Bronze 6400 EPO plan covers office visits, inpatient care, and outpatient care for mental health, behavioral health, and substance abuse treatment. You are expected to cover a 40 percent co-insurance for in-network providers.
The Cigna Health Flex 6400 plan is an LCP plan that covers in-network and out-of-network providers. A 40 percent co-insurance is required for in-network substance abuse, mental health, and behavioral health needs, while 50 percent is necessary should you stray outside it. You also need a pre-authorization for out-of-network inpatient care.
The Cigna Health Savings 6000 LCP plan requires a 0 percent co-insurance for in-network providers office visits, inpatient, and outpatient services to treat substance abuse, behavioral health, and mental health problems.
