The HMO plans have lower premiums, but have greater restrictions on the types of services you can access without a referral. With the PPO plans, you may not need a referral to see a doctor for specialist treatment for drug or alcohol dependency.

After the first four days of inpatient treatment, there is no co-insurance payment for remaining days.
