You also have the option of bronze plans that include a health savings account, which allows you to save money toward health care expenses. With the BlueEssentials 2500 HSA HDHP plan, inpatient and outpatient care are covered with a 50 percent co-insurance regardless of whether you go to a provider inside or outside the network.
On the BlueEssentials 4750 HSA HDHP plan, you spend a 10 percent co-insurance for inpatient or outpatient care, or 50 percent if you go to a provider that’s not in the network. If you have the BlueEssentials 6325 HSA HDHP plan, you have a higher deductible but a 0 percent co-insurance on inpatient and outpatient care inside and outside the network. You need certification in advance for inpatient care on any of the bronze plans.
The BlueEssentials 2500 Silver PPO plan covers outpatient services with a $30 charge for office visits and a 20 percent co-insurance for outpatient treatment. Inpatient treatment also comes with a 20 percent co-insurance and requires advance certification. For out-of-network care, you pay a 50 percent co-insurance for any kind of treatment.
The other silver option is the BlueEssentials 1900 HSA HDHP plan, which includes a health savings account. With this plan, you pay a 20 percent co-insurance for inpatient and outpatient care on the network or a 50 percent co-insurance off the network. You need prior certification for inpatient services.
At the gold level, you have the BlueEssentials 1000 Gold PPO plan. This plan covers office visits with a $20 copay and inpatient and outpatient care, you with a 20 percent co-insurance. If you prefer a center that’s outside the network, expect a 50 percent co-insurance rate for all services and certification prior to inpatient treatment.
