You have many Blue Cross Blue Shield individual health insurance plans to pick from in the state of Louisiana. You can choose from plan types and different metal levels, which designate the amount the plan pays and the amount you pay. The names of the plans include the deductible amount for staying in the network for your care. You can also find other plans with different deductibles and plan coverage. The Blue Max plans go up to the platinum level with the Blue Max Copay 80/60 $250 IND PPO plan. With this plan, you pay a $15 copay for office visits or a 20 percent co-insurance after the deductible for inpatient and outpatient treatment options. Outside the network, you would pay a 40 percent coinsurance after the out-of-network deductible instead. You may need to get authorization for your health care services.
There are also numerous Blue Saver plans that also have varying deductibles and coverage amounts, ranging from bronze to gold. One of the bronze plans is the Blue Saver 70/50 $3300 IND High plan. For in-network providers, you would pay the deductible and then a 30 percent co-insurance for both inpatient and outpatient services. For out-of-network providers, the co-insurance is 50 percent. You sometimes need to get authorization for your care. At the other end, the gold Blue Saver 100/80 $1300 IND High plan requires the deductible with no charge after that for inpatient and outpatient services when you go to treatment in the network. For out-of-network care, you pay the corresponding deductible and then a 20 percent co-insurance. You might need authorization for your treatment.
