You are responsible for a $25 copay for outpatient visits or a 20 percent co-insurance after the deductible for inpatient care. Out-of-network providers are not covered on this plan. The silver level of that plan has a $3,000 deductible. It requires a $35 copay for outpatient care, or the deductible and 20 percent co-insurance for inpatient care. It does not cover providers outside the network. The bronze level version has a $6,600 deductible for a person. The cost for inpatient and outpatient services is the deductible, and then the rest is covered. For outpatient visits, you get two visits covered 100 percent before you pay the deductible. Care outside the network is not covered on this plan.

With the BlueCare Elite PPO plan, you are subject to a 50 percent co-insurance for inpatient and outpatient services. If you go outside the network, you also have to pay an out-of-network deductible in addition to the 50 percent co-insurance. This plan has no deductible if you stay within the network.

The BlueCare Signature PPO plan comes with a $2,500 deductible for an individual if you stay in the network, or more for out of the network. For outpatient services, you would pay $35 per visit as the copay. If you choose inpatient services, you would pay a 20 percent co-insurance. If you go outside the network, you are responsible for the out-of-network deductible as well as a 40 percent co-insurance.

The BlueCare Premier PPO plan has a $1,400 deductible for a person in the network. It charges you a $25 copay for outpatient visits or a 20 percent co-insurance for inpatient care. Outside the network, you pay the out-of-network deductible and a 50 percent co-insurance.

On the BlueCare SaverPlus PPO, you would have an in-network $3,000 deductible for an individual. The cost of outpatient services is a $35 copay, while inpatient services cost the deductible plus a 20 percent co-insurance. Outside the network, you pay an out-of-network deductible and a 50 percent co-insurance.

The BlueCare Essential PPO plan has a $6,600 in-network deductible for a person. For outpatient care, you get the first two visits covered completely, then you need to pay the deductible, and then your visits after that will cost nothing. For inpatient care, you are responsible for the deductible, and then the rest is covered by the carrier. It is the same for out-of-network care except that you have a different deductible amount.

If you have the BlueCare Vital PPO plan, your individual deductible in the network is $6,600. On this plan, you pay the deductible and then the rest is covered, for both in-network and out-of-network inpatient and outpatient services. With outpatient care, you first get three visits covered completely before you’re responsible for the deductible. Also, out-of-network care has a higher deductible.
