The plans come in different metallic levels, from Bronze to Platinum. What do these levels mean? Basically, the plan will cover more of your treatment with higher level plans (Gold and Platinum), at a higher cost each month to you. You have a lower portion of care covered with lower level plans (Bronze and Silver), yet you have lower premiums each month.

Each and every individual plan will cover drug or alcohol rehabilitation treatment, in addition to mental and behavioral health concerns if you have a co-occurring condition. This means that no matter which plan you decide to sign up with, you’ll have some of your costs taken care of by your insurance company.

Delaware Individual Coverage

You’ll find many different plans in this state. The number you see in the name of each plan is a note to the individual deductible of each plan. Generally, you need to pay for the deductible before a plan provides the coverage specified for substance abuse and mental health. The following gives you some ideas of what you could expect from a Highmark plan in your state.

Bronze Level Coverage

The Bronze base plan is the HDHP Blue EPO 6850 plan, which requires no charge on your part for inpatient and outpatient services in the network. Nonetheless, you do need pre-authorization for certain types of care. Out-of-network treatment is not covered on this plan.

Silver Level Coverage

One Silver choice is the PCMH Blue EPO 2300 Base plan, which costs you a $60 copay per outpatient visit or a 25 percent co-insurance for inpatient care when you stay in the network. Out-of-network care is not covered as part of this plan. You generally need pre-authorization for your care. Another Silver option is the PCMH Blue EPO 2800 Base plan, which requires a $90 copay for each outpatient visit or a 25 percent co-insurance for inpatient services. Once again, you usually need to get pre-authorization, and out-of-network providers are not covered. You also have the Shared Cost Blue EPO 4000 Base plan to pick from. It does not cover providers outside the network, but covers in-network care at a 20 percent co-insurance cost to you. You most likely need pre-authorization for your treatment.

Gold Level Coverage

At the Gold level, you have the PCMH Blue EPO 900 plan, which costs a $60 copay for outpatient visits and a 10 percent co-insurance for inpatient treatment. You usually need to get pre-authorized, and out-of-network care is not part of your plan. Another choice is the Health Savings Blue EPO 2000 plan, which includes a health savings account. On this plan, there is no charge for in-network care, whether you choose inpatient or outpatient services. You usually need to get your care authorized in advance. Providers that are not in the network are not covered.

Platinum Level Coverage

For a Platinum plan, consider the Shared Cost EPO 300 plan. Out-of-network care is not covered. In the network, you pay a $20 copay for each outpatient visit or a 10 percent co-insurance for inpatient services. You need pre-authorization for most care.

Outside the metallic levels, a choice available to you is the Major Events Blue EPO 6850 plan. This plan requires no charge from you for in-network care and does not cover out-of-network care. Like the other plans, it generally requires you to get authorization before your care begins.
