At the bronze end, you pay less for premiums but more in copayment; that ranges to gold plans where you pay more each month but more of the cost of care is covered.

Also, each plan name includes a number, which is the deductible amount you will be responsible for. All of the Cigna individual insurance plans cover substance abuse, in addition to mental and behavioral health care if you have a dual diagnosis. With both Cigna Connect and Cigna Vantage plans, you also have the choice of variations on the listed plans. These include cost share plans and plans for Native Americans and Alaskan Natives.

Cigna Connect Plans

Many of the options in Colorado are Cigna Connect plans. At the bronze level, the Connect HSA Bronze 6000 EPO plan covers office visits, inpatient services and outpatient services with no charge on your part. The Connect Flex Bronze 6400 EPO plan only covers providers in the network, requiring you to pay a 40 percent co-insurance for all types of services.

At the silver level, the Connect Flex Silver 3000 EPO plan covers in-network providers with a 10 percent co-insurance needed on your part for office visits and inpatient and outpatient care. With the Connect Flex Silver 4000 EPO plan, you pay a $45 copay for each office visit or a 20 percent co-insurance for inpatient and outpatient care. A benefit of this plan is that the deductible does not apply to substance abuse and mental health services. The Connect Flex Silver 1500 EPO plan covers services in the network with you responsible for $60 copays for each office visit or a 30 percent co-insurance for inpatient and outpatient care.

At the gold level, the Connect Flex Gold 1200 EPO plan covers in-network providers. You cover a $40 copay for office visits and a 20 percent co-insurance for inpatient and outpatient services. With this plan, the deductible is not relevant to your substance use disorder treatment or mental health care.

For all of these plans, you need pre-authorization for inpatient care, and the plan doesn’t cover out-of-network providers.

Cigna Vantage Plans

Another choice in Colorado is a Cigna Vantage plan. At the bronze level, you have the Vantage HSA Bronze 6000 LCP plan, which requires no charge for in-network office visits and inpatient and outpatient care. On the Vantage Flex Bronze 6400 LCP plan, you pay a 40 percent co-insurance for the different types of care.

At the silver level, the Vantage HSA Silver 2700 LCP plan requires you to pay a 15 percent co-insurance for inpatient services, outpatient services and office visits. The Vantage Flex Silver 3250 LCP plan requires a $65 copay for office visits or a 30 percent co-insurance for inpatient and outpatient services. This plan includes substance use disorder and mental health as services that are covered without paying the deductible first. The Vantage Flex Silver 2000 LCP plan requires a 30 percent co-insurance for the various types of care.

The gold level option is the Vantage Flex Gold 1400 LCP plan. On this plan, you pay $45 for each office visit or a 15 percent co-insurance for inpatient and outpatient care. With this plan, the deductible does not apply to substance abuse or mental health treatment.

Pre-authorization is needed for inpatient care with every plan. None of these plans cover providers that are outside the network.
