Bronze Plans

The two Bronze Value plans both pay 100% of the cost for in and outpatient treatment if you stay in-network. If you choose a doctor or facility out-of-network, the company still pays 50% for both as long as you have met your deductible.

The Bronze Core and Basic plans change the coinsurance requirement for in-hospital detox, inpatient, and outpatient care.
